New Client Registration

Welcome to Animal Health Care Center! We have two ways you can fill out your new registration paperwork. For your convenience, we have an online form which you can fill out and submit directly to our office — just click "Submit" when you're done and the info will be on its way to us. Or, if you'd rather fill the registration out by hand, we have a printable form available. Please print, complete, and bring it in on your first visit.

If you choose the printable form, please note it must be saved to your computer before you can fill it out. You can either print it off at home or e-mail it to our clinic for us to print. Our e-mail is ahccmadison@gmail.com

Surgery Admission Form

If your pet is scheduled to have surgery with us soon, you will need to fill out a Surgery Admission Form. For your convenience, we have two ways for you to do this: you can fill out our online form and submit it directly to our office, or, if you'd rather fill it out by hand, we have a printable form available to complete ahead of time. PLEASE NOTE: We must have a correct phone number by which you or another family member/owner may be reached in case of emergency. For your pet's safety and comfort, please fill this form out as thoroughly as possible, and with all correct information.
